What this website collects
The website collects very little. The consultation request form asks for:
- Your name, so we can address you.
- Your email address, so we can reply.
- Your phone number, so we can reach you to schedule.
- Your treatment interest, a general note on what you want to discuss.
The website does not ask for, and you should not send, medical history or protected health information through the form. Do not include diagnoses, medications, or conditions in a website message. Those belong in your in-person consultation, where they are handled under the clinic's medical recordkeeping and privacy practices, separate from this website. If you submit medical details anyway, we will keep them confidential, but the website is not the right channel for them.
Why we collect it, and our legal basis
We use the information for one purpose: to respond to your request and arrange a consultation. We do not repurpose consultation-form data for marketing.
